Registered Nurse (RN)
Position Summary
Dunn-Harris Healthcare Services is seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality, client-centered care in the home setting. The RN will be responsible for delivering skilled nursing services, completing assessments, ensuring compliance with physician orders, and supporting medically fragile children and adults in accordance with Medicaid program guidelines, including GAPP, CCSP, and SOURCE.
Key Responsibilities
Clinical Care & Skilled Nursing
- Provide direct skilled nursing care in accordance with the Physician Plan of Treatment (PPOT) and care plans
- Perform ongoing client assessments and monitor changes in condition
- Administer medications and treatments as ordered
- Perform skilled procedures such as:
- G-tube care and feedings
- Tracheostomy care
- Suctioning and respiratory treatments
- Wound care
- Catheter care
- Vital signs monitoring
Documentation & Compliance
- Complete accurate, timely, and detailed nursing documentation aligned with care provided
- Ensure all documentation supports medical necessity and Medicaid compliance
- Complete required:
- Daily nursing notes
- Supervisory visit documentation
- 60-day summaries (as applicable)
- Maintain compliance with DCH, HFRD, and Medicaid program standards
Care Coordination
- Communicate effectively with families, caregivers, and interdisciplinary teams
- Report changes in client condition to physicians and office staff
- Participate in development and updates of care plans
Caregiver Education
- Educate family members on care techniques and safety
- Complete caregiver teaching and competency validation (per program requirements)
- Reinforce adherence to care plans and safety protocols
Timekeeping & EVV Compliance
- Accurately clock in/out using the agency’s EVV system (CareBravo)
- Complete all documentation during scheduled shift hours
- Ensure time worked aligns with services documented
Qualifications
- Active Registered Nurse (RN) license in the State of Georgia
